胃の内側を覆う粘膜です。粘膜内を流れる網状の血管を見ることができます。胃粘膜は胃酸や消化液によって自らが消化されないように分泌した粘液で守られています。痛み止め、解熱に使われるNSAIDsは胃の血流を阻害するほか粘液細胞の機能を直接障害して胃粘液の分泌を抑制します。NSAIDs投与によって防御能が低下した状態の胃粘膜に胃酸と同じ濃度の塩酸液を流すと、粘膜が傷害されて出血が起こります。このように防御と傷害のバランスが崩れ、粘膜が剥離することで胃潰瘍が発症します。
It shows mucosa of stomach. Reticular blood vessels stretched underneath the mucosa have been seen. Gastric mucosa with its secreting mucus is protected from gastric acid and digestive juice in order not to be digested by them. It is known that NSAIDs (Nonsteroidal Anti-Inflammatory Drugs), which provides pain-killing and antipyretic effects, cause the suppression of gastric mucous secretion by immediate dysfunctions of mucous cell besides the inhibition of blood flow in stomach. As hydrochloric acid is added in the concentration equal to gastric acid over gastric mucosa whose defense has been weakened by the administration of NSAIDs, the mucosa is being damaged and starts bleeding. In this way, when the balance between protection and aggressive factors gets upset, gastric ulcer can occur by gastric mucosa’s desquamating.
